Please accept my written notice of cancellation from my membership at VERDURE. I understand and agree, per the terms of my original membership agreement, that my notice must be submitted to VERDURE at least 30 days prior to the next billing cycle before my cancellation is effective and that I may continue to use the facilities and services until the end of said period. I understand that if I am on a term membership, I am not allowed to cancel until I have completed the full term I agreed to. I understand that I am responsible for any outstanding balance on my account and that cancellation does not eliminate balance owed.Member cannot cancel off a freeze unless the 5 month's freeze term is completed. 30 days notice is still required for Cancelling off a freeze. Cancellation must be turned in before billing cycle starts (i.e. the 1st day of the month) to be effectively cancelled for that month. If the cancellation form is submitted beyond that date, the 30 day's notice will roll to the following month's cancellation cycle. In the event you agreed to a 12-month term commitment in exchange to receive a significant Initiation Fee reduction, your membership cannot be terminated until you have completed your obligations. If you’d like to convert your membership to a month-to-month term, we will accommodate your request by having you pay the full amount of the discount you received of $200 plus a penalty for selecting a term instead of a month-to-month of $100. The total to convert is $300.
